从上述解读可以将两篇关于“利用果糖来等能量取代食物或饮料中的葡萄糖或蔗糖”研究的荟萃分析的主要结论总结如下:

总之,这两篇荟萃分析发现急性的与持续一段时间(几周内)的果糖与其它糖等量代换摄入不会对血糖与血脂指标造成不良影响。但是,因为并没有分析肝脏、腹部脂肪堆积的变化,这两篇文章关于果糖是否对肝脏脂类平衡有不良作用无法解答。不过,之前有另外的荟萃文章(Chiu et al., 2014)发现如果果糖能够严格等量代换其它糖在短期内(1-4周)也不会对肝脏脂肪产生影响,但当果糖是在常规饮食的基础上额外加入的,则十分一致地导致肝脂肪上升、肝功能受损(尽管这些变化有可能纯粹是卡路里太高造成的)!因此,关于果糖的作用,还真得看它是怎么用在饮食上的。

另外十分重要的一点是,上面提到的所有荟萃文章囊括的实验都是短期的、小样本量的,关于果糖对代谢作用真正长期的影响需要更多高质量的临床实验解答。

A:尚未有结论,有可能视情况而定。

Q:果糖能吃吗?

A:当然能。它天然存在于水果中。从水果中吃到果糖不会让你得糖尿病,不吃水果更危险。

Q:果糖是健康食品吗?

A:也不是。不但还没有确切证据说明它比其它糖好,而且不论是果糖也好,蔗糖也好,都是简单糖,都是4大卡每克的热量,吃多了总会胖。加点果糖的饮料也还是含糖饮料,营养价值一样的低。

所以说,割裂开热量和食物谈果糖,意义可能并不大。

Q:极养君推荐糖尿病人用果糖代替蔗糖吗?

A:在目前的已有认知下,不推荐。首先,果糖对血糖、脂肪代谢的影响还要更高质量的证据;其次,就算果糖真的比蔗糖、葡萄糖好那么一丁点,推荐果糖的使用也很容易被病人误解,以为这是可以无限制摄入添加糖的通行证。要知道,所有果糖可能的有效作用都是建立在它能等量替换其它糖的基础上的,与其替换,还不如都适当减少。
